Part 1: COVID-19 Screening
You are invited to reserve your spot for the Sunday Worship service using the online ticketing tool below or by phoning in to the office at (905) 723-6442.
Please reserve a place for each family member. The cut-off for reservation for each Sunday will be noon on the prior Friday.
The reservation system will help us comply with the protocols set out by the Provincial and Local Health Authorities as well as provide information for contact tracing should the need arise.
There are two parts to the reservation process. The first is a personal COVID-19 screening form designed for your own personal benefit along with other recommendations for providing a safe environment for everyone. This information will not be collected by Westminster United Church. Please review the questions to inform your decision to reserve for the upcoming Sunday Worship Service. The second is the Sunday Worship Service reservation form. In this section, we will collect your contact information as well as get an idea of how many people will be attending each service in order to plan accordingly. We will work to follow all current rules and we will re-evaluate as updates are made.
- Does anyone in your party have any of the following symptoms: fever/feverish (temperature of 37.8°C or greater), new or existing cough, shortness of breath, sore throat, difficulty swallowing, changes to sense of taste or smell, nausea/vomiting, diarrhea, abdominal pain, runny nose, or nasal congestion (not caused by seasonal allergies)?
- Has anyone in your party traveled internationally within the last 14 days (outside Canada)?
- Has anyone in your party had close contact with a confirmed or probable COVID-19 case?
If you answered NO to all of the questions above, we welcome you to attend the next available church service. If you answered YES to any of the questions above, we recommend you enjoy our recorded sermons from home.
For more detailed information on Prevention and Risk of COVID-19 and Measures to Reduce COVID-19 we recommend you consult official documentation provided by the Canadian government.
